LVN Case Manager

Heritage Provider Network is seeking a skilled LVN Case Manager to join our team. As a key member of our healthcare team, you will play a vital role in assessing, diagnosing, planning, implementing, and evaluating patient care to facilitate and coordinate the patient's plan of care with the provider.
Key Responsibilities:- Assess patient needs and develop comprehensive care plans in collaboration with the interdisciplinary care team and patient or caregiver.
- Coordinate the provision of Social Services and/or Patient Services Coordinator to patients and families to enable them to cope with the impact of illness on individual family functioning and to achieve maximum benefits from health care services.
- Strive for timely admissions and discharges from Priority Care and ensure a smooth transition of care back to Primary Care Provider after discharge from Priority Care.
- Participate in Utilization Review process regarding any assigned patients ensuring proper hand off report to UR Case Manager.
- Be knowledgeable regarding disease processes, labs, and medications to ensure the ability to assess patient needs and provide education as indicated.
- Graduation from an accredited Licensed Vocational Nursing program.
- Current California LVN license.
- Current BLS certification.
- Minimum of two years clinical nursing experience.
- Knowledge of Microsoft Office programs.
The pay range for this position at commencement of employment is expected to be between $29.33 and $34.50, however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include a sign-on bonus.
