Job Description

We are seeking a skilled Registered Nurse to join our team at Valir Health. As a Hospice Care Professional, you will be responsible for providing high-quality patient care and support to individuals and their families.

Key Responsibilities:

  • Oversee patient cases from admission to death/discharge.
  • Complete all nursing assessments for each patient assigned.
  • Maintain patient medication profiles, updating as needed, while ensuring updated profiles are in patient charts at the office, as well as in the facility or home.
  • Update the plan of care for each assigned patient every two weeks.
  • Complete the recertification process by assessing and accurately documenting findings on appropriate forms.
  • Make RN supervisory visits every 14 days on all assigned patients.
  • Complete admissions as assigned.
  • Provide on-call hours as determined by Patient Care Manager.
  • Accurately communicate patients' information to Patient Care Manager and medical staff.
  • Participate in sharing of knowledge through in-services, staff meetings, and assisting in orientation of new personnel.
  • Perform routine duties without physical supervision.
  • Recognize and respond to priorities in the patient care process.
  • Make sound judgments based on factual information, while seeking guidance and direction as needed for performance of all duties.
  • Provide support to patients and families in a courteous manner with a positive tone.
  • Assess, plan, implement, and evaluate patient care.
  • Provide nursing care to patients according to established policies and procedures.
  • Check the status of medications provided by Hospice at each visit, and reorder as needed.
  • Take and record vital signs of the patient in an accurate and precise manner.
  • Report and record any changes in condition or behavior of the patient.
  • Communicate concerns and/or changes to all disciplines.
  • Any and all other duties and responsibilities as assigned.

Requirements:

  • Excellent verbal and written communication skills.
  • Excellent interpersonal and customer service skills.
  • Excellent organizational skills and attention to detail.
  • Excellent time management skills, with a proven ability to meet deadlines.
  • Strong analytical and problem-solving skills.
  • Ability to function well in a high-paced, and at times, stressful environment.
  • Proficient with Microsoft Office Suite or related software.
  • Current Registered Nurse license.
  • Current CPR Certification.
  • Current Driver's License.
  • 1-2 years of experience in home health or hospice, LTCF preferred.
