Position Title:
Case Coordinator

Position Type:
Part-Time; 20 hours per week; availability on weekends as required

Department:
Community Support Services

Key Responsibilities:
  • Engage with designated law enforcement departments throughout the week.
  • Receive and assess referrals from participating municipal law enforcement agencies.
  • Conduct needs assessments and provide brief case management to referred individuals.
  • Assist in data collection and perform statistical analysis.
  • Collaborate effectively as part of a multidisciplinary team.
  • Conduct community outreach initiatives both independently and collaboratively.
  • Maintain and update information/data spreadsheets.
  • Coordinate with law enforcement personnel and command staff.
  • Complete necessary documentation and communicate with program staff.
  • Participate in law enforcement meetings and educational sessions.
  • Engage in co-response activities within the community alongside law enforcement officers.

Qualifications:
  • Bachelor's Degree in Social Work or a related discipline.
  • Experience in substance abuse support.
  • Proficient in computer applications including Excel, PowerPoint, and Word.
  • Understanding of law enforcement practices and procedures.
  • Bilingual candidates are preferred.
