Case Manager

Job Summary:

We are seeking a skilled Case Manager to join our team at Shore Haven. The successful candidate will provide case management services to adults diagnosed with Severe and Persistent Mental Illness (SPMI).

Responsibilities:
  • Provide case management services to assigned clients, including coordination of care and linkage to community resources.
  • Develop and implement treatment plans, including skill building, supportive, and psychoeducational groups.
  • Facilitate communication between clients, family members, and healthcare providers.
  • Monitor client progress and adjust treatment plans as needed.
Requirements:
  • Bachelor's Degree in a Human Services field.
  • Experience working with adults with SPMI.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
