About the Role

The City of East Providence, RI invites applications for a Senior Community Health Worker position. In this critical role, you will provide direct service to seniors, connecting them with essential resources, supports, and services to enhance their quality of life. You will also play a key role in developing and implementing strategies to promote healthy aging practices within our community.

Key Responsibilities:
  • Provide individualized support to seniors, addressing their physical, emotional, and social needs.
  • Connect seniors with community resources, including healthcare services, housing assistance, and nutrition programs.
  • Develop and lead workshops, groups, and other educational activities to promote healthy aging practices.
  • Collaborate with healthcare providers, community organizations, and government agencies to ensure a coordinated approach to senior care.

Requirements
  • Bachelor's degree in a related field (e.g., social work, public health, nursing).
  • Minimum 2 years of experience working in healthcare or a related field.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team.
  • Proficiency in Microsoft Office Suite and electronic medical records systems.

Compensation and Benefits
We offer a competitive salary range of $60,000-$75,000 per year, depending on experience. Our comprehensive benefits package includes medical, dental, and vision insurance, retirement savings plan, paid time off, and professional development opportunities.
