Job Summary:
We are seeking a highly skilled and organized Care Coordinator to join our team at Addus HomeCare. As a Care Coordinator, you will play a critical role in scheduling and supervising in-home care workers and clients in a geographic area.
Key Responsibilities:
- Coordinate and drive the field recruiting and hiring process.
- Oversee the new hire process for all new employees and ensure all documentation is completed timely and accurately.
- On-board and train new branch Administrative employees.
- Schedule employees as directed by client's care plan established upon intake.
- Process patient authorizations and communicate with central admissions, enter reauthorizations into client record and ensure chart preparation for all new clients.
- Create work schedules by entering schedules into the system, manage changes to client schedules due to client request, illness, vacation or leaves of absence.
- Provide alternate coverage to ensure the client's care plan is followed and client services are not interrupted.
- Supervise direct service employees by setting expectations for attendance, performance and conduct by holding employees accountable to the company's policies and guidelines.
- Assist with the new hire process for all new employees and ensure all documentation is completed accurately and in a timely manner.
Requirements & Competencies:
- High school diploma or equivalent required.
- 1 year of Industry and office experience required.
- Interpersonal, organizational and communication skills.
- Computer skills including but not limited to Microsoft Word, Microsoft Excel and Scheduling program.
- Reliable transportation.
