Job Summary:

We are seeking a skilled Behavioral Health Care Manager to develop, assess, and facilitate complex care management activities for primarily mental and behavioral health needs members. The ideal candidate will have knowledge of the case management process in a managed care or clinical setting.

Key Responsibilities:
  • Evaluate member needs via phone or in-home visits to recommend and/or facilitate care plans/services for optimal outcomes.
  • Develop ongoing care plans for members with high-level acuity, identifying providers, specialists, and community resources needed for care.
  • Coordinate between members, caregivers, community resources, and care provider teams to ensure accessible services.
  • Monitor care plans/service plans and member status/outcomes, providing recommendations based on identified member needs.
  • Facilitate care coordination and collaborate with providers/specialists to ensure timely access to needed care or services.
  • Collect, document, and maintain member information and care management activities to ensure compliance with state, federal, and third-party payer regulators.
  • Provide education to members and their families on procedures, healthcare provider instructions, treatment options, referrals, and healthcare benefits.
  • Offer feedback to leadership on opportunities to improve and enhance care and quality delivery for members in a cost-effective manner.

Must reside in Kansas. This is a hybrid position involving remote work and community-based home visits. The ideal candidate will have a Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing and 2 – 4 years of related experience. Licensed Behavioral Health Professional or RN based on state contract requirements is required.
