The ideal candidate will possess strong problem-solving skills, be self-directed, and have a working knowledge of community resources such as MA, Public Assistance, and Public Housing.
Key Responsibilities:
- Provide client-centered case management services to assist individuals in locating and gaining access to needed waiver and other state plan services.
- Coordinate client intakes, discharges, team and individual meetings, and services within set parameters and timelines.
- Identify and explore service options to meet clients' assessed needs and make referrals to outside resources based on assessment, current client need, and individualized plan.
- Evaluate and monitor services identified in the service plan.
- Administer a comprehensive assessment yearly or as needed of clients' healthcare, social needs, housing, educational, vocational, and other service needs and preferences.
- Monitor services provided to the client to evaluate the effectiveness and assure safety.
- Monitor changes in regulations in case management and waiver funding and modify services to meet those changes.
- Ensure client right to privacy, compliance with HIPAA, and all county and state regulations.
- Maintain billable time and the necessary daily records for continuity of services and reports to funding agencies.
Qualifications:
Qualified applicants must possess a Bachelor's Degree from an accredited four-year college with a degree in social work, psychology, sociology, or a closely-related field; work experience should include 1 to 3 years of experience as a social worker in a public or private social service agency.
