Family Preservation Specialist

Wellspring Lutheran Services is seeking a dedicated and compassionate Family Preservation Worker to join our team in Traverse City, Michigan.
As a Family Preservation Worker, you will play a vital role in supporting families throughout Michigan's Northwest Region, including Grand Traverse, Leelanau, Benzie, Manistee, Wexford, and Missaukee Counties.
Key Responsibilities:- Provide strength-based, non-deficit services to families, adhering to the MiFT model of service.
- Assist families in obtaining concrete services, such as housing, transportation, home cleaning, maintenance, and repair.
- Provide skills training, including parenting, self-advocacy, self-sufficiency, and communication.
- Offer short-term counseling and crisis intervention for families.
- Make collateral contacts to arrange outpatient counseling, medical care, educational needs, utility needs, and housing issues.
- Prepare documentation, including service plans, progress reports, termination summaries, goal sheets, and weekly plans.
- Present each case for termination conference with contractor, recommending follow-up services.
- Provide 24-hour availability to clients and a flexible work schedule based on family needs.
- Maintain face-to-face contact with families according to contract-defined frequency and individual needs.
- Complete follow-up evaluations of families at 3, 6, and 12 months.
- Transport clients in your own vehicle.
