Family Preservation Worker

Wellspring Lutheran Services is seeking a dedicated and compassionate Family Preservation Worker to join our team in Traverse City, Michigan.

As a Family Preservation Worker, you will play a vital role in supporting families throughout Michigan's Northwest Region, including Grand Traverse, Leelanau, Benzie, Manistee, Wexford, and Missaukee Counties.

Key Responsibilities:
  • Provide strength-based, non-deficit services to families, adhering to the MiFT model of service.
  • Assist families in obtaining concrete services, such as housing, transportation, home cleaning, maintenance, and repair.
  • Provide skills training, including parenting, self-advocacy, self-sufficiency, and communication.
  • Offer short-term counseling and crisis intervention for families.
  • Make collateral contacts to arrange outpatient counseling, medical care, educational needs, utility needs, and housing issues.
  • Prepare documentation, including service plans, progress reports, termination summaries, goal sheets, weekly plans, time reports, and letters.
  • Present each case for termination conference with contractor, recommending follow-up services.
  • Provide 24-hour availability to clients and a flexible work schedule based on family needs.
  • Maintain face-to-face contact with families according to contract-defined frequency and individual needs.
  • Complete follow-up evaluations of families at 3, 6, and 12 months.
  • Transport clients in your own vehicle.
