Job Title: Care Coordinator - Children & Family Services

We are seeking a dedicated Care Coordinator - Children & Family Services to join our team at Impact Life. This role is responsible for providing quality care management and advocacy services to clients, with a specialized focus on children's services.

Key Responsibilities:
  • Conduct individualized care management needs assessments, in partnership with the care team, to identify services needed for children and assist parents in connecting with those services.
  • Provide cultural mediation between parents and health and human services systems, ensuring effective communication and coordination of care.
  • Educate and counsel parents about plans of care, health promotion, and disease prevention, using age-appropriate language and materials.
  • Respond to calls from parents, caregivers, providers, and professionals regarding child development, behavior, or learning, and coordinate referrals and services as needed.
  • Participate in weekly case conferences and monthly team meetings to ensure care coordination and collaboration with the care team.
Requirements:
  • Bachelor's Degree in Child Development, Early Childhood Education, Human Services, Social Work, or a related field.
  • 2 years of experience working with children in a behavioral health setting, with specialized training or experience in women's studies/services, child development, and trauma.
  • Proven work experience as a Care Coordinator, Case Manager, or similar role, with strong customer service skills and effective verbal and written communication skills.
