Job Summary

Impact Life LLC is seeking a highly skilled Care Coordinator - Women's Services Specialist to join our team. As a Care Coordinator, you will play a critical role in providing quality care management and advocacy services to clients served at our Sanctuary.

Key Responsibilities

  • Evaluate women for eligibility for services within 72 hours of admission.
  • Provide culturally appropriate health education and support to clients.
  • Facilitate communication between communities and health and human services systems.
  • Educate and counsel patients about plans of care, health promotion, and disease prevention.
  • Conduct periodic updates on health status of clients according to program guidelines.
  • Educate clients and colleagues on features of health systems that promote the integration and utilization of disease prevention-health promotion services into clinical practice.
  • Meet with clients as needed to address physical health, mental health, and behavioral health in any setting conducive to the client.
  • Provide individualized client/family education customized to the client and support system.
  • Refer clients to appropriate clinical and/or medical staff as needed within the agency and in the community in an accurate and timely manner.
  • Conduct individualized care management needs assessments, prioritizing data collection based on the client's immediate condition or needs within timeframe specified by client facility's policies, procedures.
  • Conduct ongoing assessments as determined by the client's condition and/or the client facility's policies, procedures or protocols and reprioritize care accordingly.
  • Provide resources and coordinate referrals as necessary and required.
  • Provide transportation to appointments.

Requirements

  • Bachelor's Degree in Human Services, Social Work, or related field.
  • Certified Alcohol and Drug Counselor (CADC) preferred.
  • 2 years proven work experience as a Care Coordinator or similar role.
  • Minimum 2 years of work experience in the human services field.

  • Highly organized with a strong ability to manage different priorities.
  • Ability to work with others and use resources to problem solve.
  • Apply good judgment to fast-changing situations.
  • Strong customer service skills.
  • Effective verbal and written communication skills.
  • Specialized training or experience in:
    • Women's studies/services.
    • Trauma-informed care.
    • Public health practices.
    • Disease prevention.
    • Healthcare systems.
    • Harm reduction models.
