Licensed Practical Nurse II

Ellis Medicine is seeking a skilled Licensed Practical Nurse II to join our team. As a Licensed Practical Nurse II, you will play a vital role in providing high-quality patient care and contributing to the overall success of our healthcare organization.
Job Summary:The Licensed Practical Nurse II will provide supportive and restorative care to patients under the supervision of a registered professional nurse. This includes basic patient care responsibilities, such as hygiene and comfort needs, nutrition and fluid balance, and medication administration. The ideal candidate will have strong communication and collaboration skills, with the ability to work effectively in a fast-paced environment.
Responsibilities:- Provide basic patient care, including hygiene and comfort needs, nutrition and fluid balance, and medication administration.
- Assist patients with activities of daily living, such as bathing, skin care, feeding, and ambulation.
- Monitor patient safety and skin condition, and take appropriate action to prevent harm.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care delivery.
- Participate in data collection and reporting to aid in ongoing development of patient plans of care.
- Graduation from an accredited LPN program.
- Current licensure in New York State or eligibility for a limited permit to practice.
- Documentation of successful completion of pharmacology coursework.
- Current BLS certification.
- No previous experience required.

- Compassion: Serve every person with empathy, dignity, and compassion.
- Ability to push/pull, lift/move 35 pounds, and perform moderately difficult manual manipulations.
- Ability to perform tasks requiring hand-eye coordination, such as data entry and typing.
- Ability to be stationary at a workstation for prolonged periods and move around the unit as needed.
The Licensed Practical Nurse II will participate in data collection and reporting to aid in ongoing development of patient plans of care. This includes:
- Interviewing patients and families to collect pertinent data.
- Monitoring patients as directed by the RN and communicating findings in a timely manner.
- Accurately obtaining and recording vital signs as ordered and reporting significant variations in a timely manner.
- Demonstrating knowledge of age-specific ranges and techniques to obtain vital signs.
- Providing information on patient progress to the RN to aid in plan of care revision.
- Mandatory education records will be completed and forwarded to Human Resources annually.
- Annual health assessment forms will be completed and maintained current inoculations.
- Current licensure as required by N.Y. State will be forwarded to Human Resources upon renewal or issuance.

Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
