Job Title: RN Home Care

We are seeking a skilled Registered Nurse to join our team in providing high-quality home care services to our residents. As a RN Home Care Specialist, you will be responsible for delivering nursing care according to established policies and procedures for a Medicare-certified home care agency.

Key Responsibilities:
  • Partner with residents, families, and healthcare professionals to develop and implement individualized care plans.
  • Provide nursing care as outlined in the plan of care and according to acceptable nursing standards.
  • Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in care conferences and other resident-related meetings to ensure comprehensive care.
  • Collaborate with the Home Health and Home Support agency team to ensure effective care delivery.

Requirements:
  • Graduate of an accredited Registered Nurse degree program.
  • Current Registered Nurse license for the State of Maryland.
  • Current CPR certification, required.
  • Current OASIS experience preferred.
  • Home Care experience preferred.
