Care Manager LCSW FT/Days

Care Management within the Department of Psychiatry at Providence Health & Service involves a collaborative practice with patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners, and the community.
Social workers facilitate communication and care coordination along a continuum through effective transitional care management.
Recognizing the patient's right to self-determination, the significance of the social determinants of health, and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.).

Master's Degree - Social Work is required.
Our ideal candidate will have:
- 2 years of experience in a healthcare-related setting with a psychiatric mental health patient population in the area of service, preferably in an inpatient setting.
- Computer experience.
- Discharge planning experience.
Additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
