Hospice Aide

Care Partners, a renowned non-profit hospice provider, is seeking a dedicated and compassionate Hospice Aide to join our team. As a Hospice Aide, you will play a vital role in delivering high-quality, patient-centered care to individuals and their families in the Portland metropolitan area.
Responsibilities:- Provide personal care to patients in accordance with their individualized plan of care and visit frequency.
- Assist patients with bathing, oral care, skin care, and hair care, as well as ambulation and positioning for comfort.
- Perform patient turns and change bed linens as needed.
- Report changes in patient condition and needs to the care team.
- Maximize patient comfort, safety, and health consistent with hospice philosophy and organizational policies.
- Maintain accurate and up-to-date patient records.
- A minimum of one year of prior CNA, Hospice Aide, or equivalent work experience.
- A current certificate of practice as a Certified Nursing Assistant (CNA) in the state of Oregon.
- Basic Life Support CPR card for Health Care Provider and First Aid certification.
- A valid driver's license and acceptable driving record, with access to a reliable automobile.
- Excellent communication and documentation skills, with the ability to work successfully with patients and family members.
- Proficiency in electronic tools, including email, internet, and keyboarding, with the ability to learn medical software.
- Reliability and ability to work independently and as part of a team.
- Ability to lift and move patients, with frequent walking and standing.
- Bi-lingual, Spanish-speaking ability is a plus.
