Role SummaryThe STAR Coordinator is responsible for overseeing the care of individuals enrolled in the Support Transition and Reentry (STAR) program. This initiative focuses on assisting adults transitioning from correctional facilities who may also face behavioral health or developmental challenges. The Coordinator will create and maintain a comprehensive Transition Plan aimed at fostering self-sufficiency through established services and ongoing support.
Key Responsibilities- Manage care coordination for participants in the Transitions to Community Living program.
- Collaborate effectively with STAR Specialists.
- Implement transition planning with an emphasis on quality outcomes, utilizing evidence-based practices and national care standards.
- Engage participants throughout the transition process.
- Develop tailored discharge and transition plans, including supported housing and community outreach.
- Collect and analyze outcome data to measure effectiveness.
- Educate participants about available mental health, substance use, and developmental disability services, as well as community resources.
- Ensure thorough documentation of all activities and plans.

Education: Associate's or Bachelor's degree
Experience:
- Associate's degree with four years of relevant experience, or
- Bachelor's degree with two years of relevant experience, or
- Equivalent combination of education and experience.
Preferred Experience:
- At least one year of experience in criminal or juvenile justice systems.
- Demonstrated ability to work with diverse populations across various socio-economic and cultural backgrounds.
Licensure/Certification: None required, but a valid driver's license is necessary.
Location Requirement: Candidates must reside in North Carolina to be eligible for remote work.
