About the Role

Key Responsibilities
  • Evaluate member needs and identify barriers to accessing care, social determinants of health, and prioritize member-identified needs.
  • Develop and implement ongoing care plans, collaborating with providers to identify resources and specialists to address member needs.
  • Coordinate between members, caregivers, and care providers to ensure person-centered care and services.
  • Provide ongoing follow-up and monitoring of member status, progress towards care plan goals, and revise plans as necessary.
  • Facilitate care management and collaborate with providers to ensure timely access to needed care or services.
  • Collect, document, and maintain member information and care management activities to ensure compliance with regulations.
  • Provide education to members and caregivers on disease processes, care options, referrals, and healthcare benefits.
  • Collaborate with leadership to identify opportunities to improve and enhance care delivery for members.
Requirements
  • Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2 – 4 years of related experience.
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
