- Develop and implement ongoing care plans, collaborating with providers to identify resources and specialists to address member needs.
- Coordinate between members, caregivers, and care providers to ensure person-centered care and services.
- Provide ongoing follow-up and monitoring of member status, progress towards care plan goals, and revise plans as necessary.
- Facilitate care management and collaborate with providers to ensure timely access to needed care or services.
- Collect, document, and maintain member information and care management activities to ensure compliance with regulations.
- Provide education to members and caregivers on disease processes, care options, referrals, and healthcare benefits.
- Collaborate with leadership to identify opportunities to improve and enhance care delivery for members.
- Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2 – 4 years of related experience.
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
