Care Transition Navigator

We are seeking a highly skilled Care Transition Navigator to join our team at Methodist Health System. As a Care Transition Navigator, you will play a critical role in coordinating activities that promote quality outcomes, patient throughput, and discharge planning.
Key Responsibilities- Coordinate patient care transitions to ensure seamless handoffs between healthcare providers
- Identify potential barriers to patient throughput and quality outcomes, and develop strategies to mitigate them
- Collaborate with interdisciplinary teams to develop and implement effective care plans
- Communicate clearly and effectively with patients, families, and healthcare providers to ensure informed decision-making
- Monitor and analyze patient outcomes to identify areas for improvement
- Bachelor's degree in Social Work, Master's degree in Social Work, or Registered Nurse with BSN preferred
- 1 year of related work experience in a healthcare setting
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize multiple tasks
