Abilene, Texas, United States Texas Department of Aging & Disability Services Full time
Job Overview:

Advanced Practice RN I

Experience a workplace that genuinely appreciates and honors the individuals we support, believing in the vision that everyone can attain a greater level of health and autonomy.

The state-supported living centers cater to individuals with intellectual and developmental disabilities who may be medically vulnerable or exhibit behavioral challenges.

These centers deliver campus-based direct services and assistance across 13 locations statewide.

The state of Texas provides numerous advantages to its workforce.

HHSC qualifies for the Federal Public Service Loan Forgiveness Program, which eliminates the remaining balance on Direct Loans after fulfilling specific monthly payment criteria.

We also offer outstanding health benefits, a lifelong monthly retirement annuity, as well as 401(k) and other programs under the Employees Retirement System of Texas.

If you seek a career filled with purpose, this position is ideal for you.

The Advanced Practice RN is appointed by the Medical Director with the approval of the SSLC Director. This role involves delivering advanced clinical medical care under a physician's supervision. Responsibilities include assessing, planning, and providing comprehensive patient care either independently or in collaboration with other healthcare professionals. You will oversee medical activities/treatments, monitor trends, and propose enhancements. Treatment is also provided to HHS employees injured during the course of their work.

Additional duties as assigned by the Medical Director/Facility Director may include planning, assigning, and/or supervising the work of others.

Operate with minimal supervision while exercising considerable initiative and independent judgment. Clinical and administrative oversight is provided by the Medical Director and/or other supervising physicians.

Shift details (if applicable): This role may qualify for additional compensation for work performed during evenings, nights, and/or weekends.

Key Responsibilities:

Regular attendance is expected, and you may be required to adhere to a specific shift schedule or, at times, a rotating schedule, extended shifts, and/or overtime as per agency leave policies, along with other assigned duties.

You may also participate in an on-call rotation as necessary to meet the operational needs of the facility.

Deliver acute care to individuals, which includes daily assessments for urgent health issues or injuries in the clinic and/or infirmary.

The assessment process encompasses diagnosis, treatment, and care planning with documentation of acute concerns, evaluating injuries, including potential sutures and/or referrals to hospitals or emergency services.

Provide chronic care for individuals, which involves quarterly assessments for those with identified chronic health conditions, including reviewing appropriate treatments specific to their medical needs and updating or modifying medical care as necessary.

Conduct comprehensive medical care, which includes making rounds, reviewing the status of patients in the infirmary and hospitalized cases, and on-call situations.

Offer medical diagnoses and review treatment plans during the Individual Service Plan (ISP) meetings.

This includes evaluating all identified risks for individuals (such as choking, pneumonia, etc.) and ensuring that necessary supports (OT, PT, Nursing, etc.) are recognized and addressed.

Develop precise and timely documentation, which includes dictation of annual summaries, pre-procedural histories and physicals, specialty medical consultation records, and all acute and chronic patient care documentation.

Provide treatment to HHS employees injured during the course of their work as needed.

Other responsibilities may include actively participating and/or supporting the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such involvement may necessitate alternate shift patterns and/or locations.

Required Knowledge, Skills, and Abilities:

Familiarity with methods and procedures in general medicine/nursing, including laws and regulations governing public health and hospital operations.

Knowledge of principles, methods, and procedures for delivering medical evaluations, diagnoses, and treatments.

Understanding of the principles involved in developing, implementing, and documenting case histories and individualized care plans.

Proficiency in interpreting laboratory analyses and x-rays.

Competence in patient care and in utilizing medical diagnostic and treatment tools and equipment.

Ability to examine, diagnose, and treat physical and mental health disorders.
Capability to supervise the work of others providing medical care to patients.
Ability to interact appropriately with individuals with developmental disabilities.
Proficiency in presenting ideas clearly and concisely in both written and oral communications.
Ability to prepare and maintain medical records.
Ability to collaborate effectively with professionals and paraprofessionals as a productive member of an interdisciplinary team.
Ability to work independently and adapt flexibly to assigned responsibilities.
