About This Role

We are seeking a highly skilled Social Worker to join our team at Methodist Le Bonheur Healthcare. As a Social Worker, you will play a critical role in coordinating patient-centered care and service delivery to facilitate optimal transitions and progression in care.

Key Responsibilities
  • Coordinate and collaborate with patients, caregivers, physicians, and other healthcare professionals to ensure seamless care transitions.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Provide education and support to patients and families to promote self-advocacy and empowerment.
  • Collaborate with the interdisciplinary team to identify and address social determinants of health that impact patient outcomes.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
Requirements
  • Master's Degree in Social Work (MSW) from an accredited program.
  • 5-7 years of experience in a healthcare setting, with a minimum of 2 years in an inpatient or acute care setting.
  • Licensed Clinical Social Worker (LCSW) or Licensed Medical Social Worker (LMSW) certification.
  • ACM/CCM certification preferred.
Preferred Qualifications
  • Experience working with diverse patient populations, including those with complex medical and social needs.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Knowledge of community resources and social determinants of health.
