**Job Description:**
We are looking for a skilled LPN who can provide high-quality care to assigned clients based on their physical, psychosocial, emotional, and spiritual needs. The ideal candidate will have excellent communication and interpersonal skills, as well as experience working with older adults and their families.

The successful candidate will be responsible for implementing programming, completing client health screenings, assessing client strengths and needs, and developing client care plans. They will also advocate for clients' healthcare needs, manage ongoing care and service provision, and collaborate with the interdisciplinary team to ensure comprehensive care delivery.

**Responsibilities:**
• Deliver high-quality care to assigned clients based on their physical, psychosocial, emotional, and spiritual needs.
• Implement programming for assigned clients, including health screenings and evidence-based practice guidelines.
• Assess and identify client strengths, needs, concerns, and preferences.
• Develop client care plans that identify goals and resources to facilitate wellness.
• Implement care and service plans based on qualified assessments.
• Advocate for an individual's healthcare needs and coordinate care to minimize healthcare fragmentation.
• Collaborate with the interdisciplinary team to ensure comprehensive care delivery.
• Practice care management professionally.
• Demonstrate comprehension and application of theories related to human behavior, crisis, family systems, personality, change, and adult learning.

The estimated salary for this position is $67,500 - $77,500 per year.
