Job Summary

We are seeking a highly skilled and compassionate Transitional Living Services Manager to join our team at Youth Homes - Montana. As a key member of our staff, you will be responsible for managing a caseload of youth and young adults ages 15-21 who are transitioning out of the foster care system.


Key Responsibilities
  • Case Management: Develop and implement individualized plans to support the educational, employment, and independent living goals of our clients.
  • Service Coordination: Collaborate with community resources and providers to ensure seamless delivery of services and support.
  • Transition Planning: Work closely with clients to identify and achieve their long-term goals, including securing stable housing, employment, and education.

Requirements
  • Education: Bachelor's Degree in Human Services or a related field.
  • Experience: At least 2 years of experience working with high-risk youth and young adults in foster care and their families.
  • Licensure: Valid Montana Driver's License.
  • Work Schedule: 40 hours per week.
