Critical Care Navigator

Dartmouth-Hitchcock Concord is seeking a skilled Critical Care Navigator to join our team in the Capacity Coordination Center. As a Critical Care Navigator, you will play a crucial role in ensuring the smooth transfer of patients between levels of care during their inpatient hospitalization. This is an excellent opportunity for a dedicated and detail-oriented individual to make a meaningful impact on patient care.
In this role, you will work closely with the On-Call Medical Director and other members of the multidisciplinary team to develop and implement strategies that maximize safe patient intake and resource needs. Your ability to communicate effectively with referring providers and DH staff, as well as your expertise in electronic patient placement systems, will be essential to your success in this position.
The successful candidate will be responsible for coordinating patient transfers, prioritizing activities associated with accurate transfer timing, placement needs, and bed assignment. You will also work to identify and delegate appropriate tasks to assistive personnel/coordinators and develop long-term relationships with house supervisors, charge nurses, referring facilities, and providers. A strong understanding of DHH services and policies will be beneficial in this role.
This is a full-time opportunity with a salary range of $110,000 - $130,000 per year, depending on experience. Required qualifications include a graduate degree from an accredited Nursing Program and 3 years of critical care experience. Leadership experience is desirable. A Basic Life Support (BLS) Certificate is required, and licensure as a Registered Nurse in New Hampshire is necessary. Previous computer experience, particularly with Word, Windows, and Excel, is required. Proven organizational and multi-tasking abilities, along with strong customer service skills and excellent phone etiquette, are essential for success in this role.
