About the Role

We are seeking a highly motivated and experienced Registered Nurse (RN) to join our Capacity Coordination Center team. As a Senior Critical Care Navigator, you will play a crucial role in ensuring seamless patient flow and coordinating critical care services across the Dartmouth-Hitchcock Health system.

Key Responsibilities
  • Obtain and document relevant clinical data to assess patient needs and ensure appropriate level of care.
  • Act as a navigator for referring providers to identify and access appropriate DHH services.
  • Manage and coordinate communications between referring and DH providers via telephone and electronic communication methods.
  • Identify and facilitate patients for redirect and placement to DHH affiliates.
  • Determine appropriateness of requests for outside transfers and prioritize activities associated with accurate transfer timing and placement needs.
Requirements

To be successful in this role, you will require:

  • A graduate degree from an accredited Nursing Program.
  • A minimum of 3 years of critical care experience, preferably with leadership experience.
  • Strong organizational and multi-tasking abilities, along with excellent customer service skills.
  • Proven ability to work effectively in a high-stress, fast-paced environment.
  • Current licensure as a Registered Nurse with NH eligibility.
  • BLS Certificate required.
What We Offer

We offer a competitive salary range of $85,000 - $115,000 per year, depending on experience, as well as a comprehensive benefits package and opportunities for professional growth and development.
