Job Summary

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient needs and develop effective care plans.
  • Collaborate with healthcare teams to coordinate care and ensure seamless transitions.
  • Provide crisis intervention, grief and loss counseling, and discharge planning to support patients and families.
  • Stay up-to-date with federal and state regulations related to social work practice.
Requirements
  • Master's degree in social work (MSW) from an accredited program.
  • State of Michigan Social Work License (LMSW) or equivalent.
  • Experience in individual, family assessment, crisis intervention, and discharge planning preferred.
