As a key member of our interdisciplinary team, you will work collaboratively with patients, families, and healthcare professionals to address complex psychosocial needs and ensure seamless transitions of care.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient needs and develop individualized plans
  • Collaborate with healthcare teams to coordinate care and ensure timely discharges
  • Provide crisis intervention, grief and loss counseling, and other support services as needed
  • Develop and maintain knowledge of community resources and provide referrals to patients and families
  • Participate in departmental committees and projects to advance social work practice and patient care
Requirements
  • Master's degree in social work (MSW) from an accredited program
  • State of Michigan Social Work License (LMSW) or eligibility for licensure
  • Experience in individual, family assessment, crisis intervention, and discharge planning preferred
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
