The Clinical Assessment Nurse plays a vital role in the healthcare delivery system by collaborating with the Care Management team to evaluate members' eligibility for various programs offered by Centers Plan for Healthy Living. This position involves conducting thorough assessments in the member's residence, utilizing approved assessment tools to ensure accurate evaluations.
Key Responsibilities:
- Conduct comprehensive assessments using the UASNY tool, including functional and mental health evaluations to determine program eligibility.
- Assess member risk levels based on detailed evaluations and provide face-to-face initial care visits as needed.
- Identify and address the medical, functional, psychosocial, and behavioral health needs of members, ensuring active engagement in the development of personalized care plans.
- Complete all necessary documentation accurately and within established timeframes, adhering to organizational policies and regulatory guidelines.
- Collaborate effectively with Care Managers to ensure continuity of care and address any urgent member needs.
- Participate in interdisciplinary team meetings to discuss care quality and escalate any concerns appropriately.
- Maintain confidentiality and comply with HIPAA regulations while participating in quality improvement initiatives.
Qualifications:
- Active and unrestricted RN license in New York State.
- Strong communication skills, both written and verbal.
- Valid driver's license and access to reliable transportation.
- Experience in home care settings and managed care environments is preferred.
- Computer proficiency is required.
