About the Role

We are seeking a highly skilled and dedicated MDS Coordinator to join our team at Lubbock Health Care Center. As a key member of our staff, you will play a vital role in ensuring the highest quality of care for our residents.

Key Responsibilities
  • Schedule residents for assessments and ensure compliance with federal and state regulations.
  • Monitor and maintain accurate and up-to-date documentation of resident care plans.
  • Coordinate with nursing and other departments to ensure seamless communication and care delivery.
  • Attend regular Medicaid meetings and ITTM meetings to discuss resident care and updates.
  • Perform transmissions of the MDS on a timely basis and provide reports to management.
Requirements
  • Licensed Nurse in good standing.
  • Geriatric nursing background and restorative care philosophy.
  • Excellent communication, organization, and time management skills.
  • Ability to work independently and as part of a team.
