About the Role

We are seeking a highly skilled MDS Coordinator to join our team at Lubbock Health Care Center. As a key member of our healthcare team, you will play a critical role in ensuring the highest quality of care for our residents.

Key Responsibilities
  • Schedule residents for necessary assessments and ensure timely completion of the comprehensive assessment (MDS).
  • Monitor and maintain accurate and up-to-date documentation of resident care plans and MDS.
  • Collaborate with nursing and other departments to ensure seamless communication and coordination of care.
  • Stay current with industry changes and best practices through ongoing education and training.
  • Perform other related duties as assigned.
Requirements
  • Licensed Nurse in good standing.
  • Geriatric nursing background and experience.
  • Excellent communication, organizational, and time management skills.
  • Ability to work independently and as part of a team.
  • Working knowledge of nursing home regulations and industry standards.
