Job Title: MDS Coordinator

We are seeking a skilled MDS Coordinator to join our team at Lubbock Health Care Center. As a key member of our healthcare team, you will be responsible for coordinating the comprehensive assessment process, ensuring compliance with federal and state regulations, and maintaining accurate and up-to-date documentation.

Responsibilities:
  • Schedule residents for assessments and ensure timely completion of the comprehensive assessment process
  • Monitor and maintain accurate and up-to-date documentation of resident care plans and MDS assessments
  • Coordinate and attend regular Medicaid meetings to discuss resident care and updates
  • Perform care plan updates as needed and provide reports to the DON and Administrator
  • Remain current with industry changes and best practices through attendance at workshops and conferences
Requirements:
  • Licensed Nurse in good standing
  • Geriatric nursing background and experience
  • Excellent communication and organizational skills
  • Working knowledge of nursing home regulations and industry standards
