Job Overview

Care Advantage Inc. Corporate is seeking a skilled Home Care Supervisor to join our Personal Care Team. As a key member of our team, you will be responsible for overseeing the delivery of high-quality care services to our clients in the Central/Western MD area.

Responsibilities
  • Develop and implement individualized care plans for clients
  • Lead and manage a team of caregivers to ensure excellent patient outcomes
  • Conduct regular assessments and evaluations to ensure client needs are met
  • Collaborate with healthcare professionals to ensure seamless care transitions
  • Monitor and report on client progress and any changes in their condition
  • Provide education and training to caregivers on best practices and new developments in care
  • Ensure compliance with company policies and procedures
  • Perform other duties as assigned by the Administrator
Requirements
  • Active Maryland Registered Nurse license
  • Minimum 1 year experience in a nursing role (LPN to RN acceptable)
  • Associate's Degree in Nursing
  • Excellent communication and leadership skills
  • Ability to work collaboratively in a team environment
  • Computer/iPad technology skills for documentation purposes
