Job Title: Group Home Supported Community Living Specialist

Job Summary:

Nishna Productions Inc is seeking a highly motivated and dedicated Group Home Supported Community Living Specialist to join our team. As a key member of our residential services team, you will be responsible for providing quality care and support to individuals with disabilities in a group home setting.

Responsibilities:

  • Teach, train, and assist individuals in developing skills necessary for independence in daily living activities.
  • Coordinate health and medical needs, and train individuals to take care of their own health and medical needs.
  • Develop or maintain skills allowing for better participation in the community.
  • Assist individuals in accessing community services and participating in at least one community activity per week.
  • Provide emergency/safety skills training and assist individuals in advocacy procedures to ensure their rights and meet their needs, interests, and choices.
  • Prepare individualized diet plans and provide training in skills necessary for managing personal business and property.
  • Provide transportation and training in daily living skills, including meal preparation, laundry, cleaning, and personal hygiene.
  • Supervise individuals' goals and complete data collection related to running those goals.
  • Supervise individuals' finances and provide guidance in budgeting for personal needs and desires.
  • Provide a supportive and therapeutic atmosphere and supervise individuals' care and use of personal property.
  • Train new staff relative to duties, work schedules, agency policies, and state and/or federal regulatory policies and procedures.
  • Follow the assigned work schedule and never leave individuals receiving 24-hour care unsupervised.
  • Complete all records and documentation using the proper forms as they relate to established procedures.
  • Attend staff meetings and scheduled in-service training sessions.
  • Ensure the individual's residence meets cleanliness, safety, and health standards.
  • Keep all written and verbal information on each individual strictly confidential.
  • Complete documentation of behaviors or provide information for writing staff intervention plans.
  • Handle any other duties as assigned by the Team Leader, Lead Staff, and Director of Residential Services.

Requirements:

  • High school diploma or GED.
  • Previous similar job experience in teaching skills of cooking, cleaning, and personal hygiene.
  • Exercise sound judgment when the situation requires immediate decision-making.
  • Obtain and maintain a valid Iowa Chauffeur's license and maintain an acceptable driving record.
  • Successful completion of pre-employment physical, TB Tine, and drug screenings prior to employment.
  • Complete 10-hour med-management class.
  • Must be willing to work with individuals who have a behavioral disorder, which may result in the display of aggressive outbursts of a physical nature.
  • Must be willing to clean any biological fluids/waste.
  • Must have the ability and desire to work with other employees as a team.
  • Must have good verbal and written communication skills.
  • The Supported Community Living Specialist position requires one to be self-motivated.
  • Must not have been legally convicted of any type of assault, abuse, or bodily injury.

Physical Requirements:

  • Must be able to travel from place to place, up to 70 miles.
  • Must be able to enter and exit resident homes that may or may not be accessible to everyone.
  • Must be able to walk up and down stairs with hands full to perform laundry duties, etc.
  • Must be able to work a full scheduled shift as assigned.
  • Must be able to carry necessary supplies up to 50 pounds to perform job duties.
  • Must be able to provide physical care necessary to residents when performing direct care duties.
  • Ability to physically assist an individual to regain their balance or to transfer from a sitting, standing or prone position.
  • Demonstrate ability to perform routine household tasks, such as, but not limited to vacuuming, mopping, and the use of cleaning agents, meal preparation, kitchen clean up, and clothing care.
  • Must be able to assist an individual in a wheelchair to transfer to another chair, bed, toilet, or bath.
  • Must be able to assist someone in the shower or bath.
  • Must be able to make multiple trips from car to house carrying purchases such as groceries, etc.
  • Must be able to perform snow removal as much as necessary for individual and staff safety.
