Salary: $85,000 - $115,000 per year, depending on experience and qualifications.

Job DescriptionYou will conduct in-person patient interviews, perform comprehensive physical assessments, and oversee the implementation and ongoing assessment of the patient's plan of care through management of home health aides, LPNs, RNs, and other caregivers. You will communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve. Additionally, you will provide education to patients and families on proper home health care procedures, such as wound care, IV administration, and medication management.

Required Skills and Qualifications

You must have a current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position. Current CPR/AED/BLS/First Aid certification is also required. You should have reliable transportation to/from care sites and/or work locations. A minimum of three years of home health care experience and two years of management experience are necessary for this role.
