Role Responsibilities
- Evaluate and address barriers to care, involving patients and families in developing potential solutions while considering their physical and emotional well-being, health literacy, and cultural background.
- Identify and coordinate internal and external resources to meet patient needs, continuously enhancing knowledge of community resources, including those related to immigration, employment, and financial assistance.
- Educate patients and caregivers about the multidisciplinary nature of cancer care and survivorship, clarifying the roles of team members and what to expect from the healthcare system.
- Empower patients by promoting self-management skills and providing information on available resources and referrals.
- Facilitate effective communication between patients, families, and healthcare providers to optimize patient outcomes.
- Assess patients' ability to advocate for themselves, helping them prioritize questions and clarify information during interactions with their care team.
- Contribute to the development, implementation, and evaluation of the patient navigation program, completing special projects and tasks as assigned by management.
- Demonstrate empathy and provide information and support within the scope of practice, referring to licensed clinicians when necessary.
- Adhere to organizational policies and procedures to ensure compliance with regulations, including HIPAA, and document care provided in accordance with established protocols.
Qualifications
Required:
- High school diploma.
- A minimum of three years of experience in social services, healthcare, or a related field.
- Strong communication skills, with the ability to engage effectively and courteously with patients and families from diverse cultural backgrounds.
- Proven ability to collaborate with a multidisciplinary team.
- Excellent organizational skills and proficiency in both written and oral communication.
- Ability to work independently, adapt to changing tasks, and prioritize effectively.
- Team-oriented with a commitment to timely communication and reliability.
- Fluency in English, both spoken and written.
- Familiarity with community resources in the greater Puget Sound Region.
- Technical proficiency in Microsoft Office and relevant software programs.
Preferred:
- Bachelor's degree.
- Fluency in a language other than English is advantageous.

The hourly pay range for this position is competitive and based on experience and qualifications.
