About the Role

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Haven Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals and their families.

Responsibilities
  • Provide comprehensive medical care, including evaluation, assessment, and management of terminal illnesses, in compliance with state Nurse Practice Act and under the supervision of a collaborating physician.
  • Conduct thorough history and physical examinations to assess patients' medical conditions and health histories.
  • Complete hospice face-to-face visit requirements to assess eligibility for certification and recertification for hospice care.
  • Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
  • Maintain accurate and detailed clinical notes, ensuring compliance with all state and federal legal and regulatory requirements.
  • Communicate effectively with physicians and participate in interdisciplinary group meetings to meet the medical and emotional needs of patients and their families/caregivers.
Qualifications
  • Graduate from an accredited School of Nursing and accredited Nurse Practitioner program.
  • Current DEA license, if applicable to state regulations.
  • Current nursing license in the state of practice.
  • Current CPR certification.
  • Master's degree with a minimum of one year NP experience.
  • One year of hospice or palliative care experience preferred.
  • Accreditation from an approved certifying body for advanced practice nursing as required by state of practice.
  • Certification in a specialist area preferred (e.g., Hospice and Palliative Nursing (CHPN) or Geriatrics).
