Community Hospice of Northeast Florida is seeking a compassionate and skilled Hospice Aide/CNA to join our team. As a Hospice Aide/CNA, you will play a vital role in providing high-quality patient care to terminally ill individuals in various settings.
Job Summary:The Hospice Aide/CNA will work under the supervision of the Regional Director or designated RN to perform basic patient care activities necessary for the personal needs and comfort of terminally ill patients. This includes assisting patients with daily living activities, maintaining a safe patient environment, and providing emotional support.
Responsibilities:- Perform personal care activities to promote and maintain patient hygiene.
- Assist patients with ambulation and exercise.
- Maintain a safe patient environment to prevent patient injury.
- Provide care in an emotionally supportive approach.
- Keep Interdisciplinary Group (IDG) members abreast of patient and caregiver status, changes, and or concerns.
- Provide nutritional support to patients.
- Assist in post-mortem care.
- Participate as an active member of IDG and participate in meetings, when appropriate.
- Attend and participate in regularly scheduled staff meetings.
- Work with all Community Hospice departments to ensure compliance with Community Hospice objectives, plans, and procedures.
- Maintain a neat and clean environment while carrying out all duties.
- Provides light housekeeping, and meal preparation as directed in the patient plan of care.
- Demonstrate to family/caregiver proper techniques to meet the personal care needs of the patient.
- Reports changes in patient's condition or care needs that require follow-up to supervisory and/or appropriate team member.
- Documents care provided in accordance with organization standards.
- Maintains a minimum of twelve (12) hours of training/education specific to position annually.
- Certified Nursing Assistant (CNA) license, State of Florida.
- State of Florida Driver's license with driving record acceptable to Community Hospice insurance carrier; reliable transportation.
- Current AIDS 104 Certification.
Typical qualifications would be equivalent to:
- High school diploma or GED equivalent preferred.
- Six (6) months experience in home care, hospital, and/or long term care.
- Experience in caring for the terminally ill preferred.
- Reliable transportation that allows employees to shift work locations when needed.
