We are seeking a skilled and compassionate Hospice Nurse Practitioner to join our team at Haven Hospice, a part of BrightSpring Health Services. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals with serious illnesses, focusing on comfort, dignity, and quality of life.
Responsibilities- Provide comprehensive medical care, including evaluation, assessment, and management of patients' medical conditions
- Develop and implement individualized plans of care, taking into account patients' unique needs and goals
- Collaborate with healthcare teams to ensure seamless care coordination and communication
- Conduct regular assessments and evaluations to ensure patients' needs are met and care plans are effective
- Document patient interactions, assessments, and care plans accurately and thoroughly
- Master's degree in Nursing with a focus on Advanced Practice Registered Nursing (APRN)
- Current DEA license, if applicable to state regulations
- Current nursing license in state of practice
- Current CPR certification
- Minimum of one year NP experience, with preference for hospice or palliative care experience
- Accreditation from an approved certifying body for advanced practice nursing as required by state of practice
