Job Title: Hospice LVN Per Diem Position

We are seeking an experienced Hospice LVN to provide compassionate end-of-life care for our patients and their families in the Inland Empire area.

Job Summary:

The ideal candidate will have excellent communication and time management skills, with an emphasis on superior bedside manners. The key component of this position is schedule flexibility to accommodate patient visits on a weekly/biweekly frequency.

Responsibilities:
  • Coordinate with assigned patients to schedule visits at the ordered frequencies.
  • Communicate with the hospice team regarding patient needs.
  • Provide education and support to patients and their families.
  • Monitor patient response to interventions and adjust care plans as needed.
  • Develop effective working relations and cooperation with the hospice team throughout the entire case management process.
  • Maintain accurate and timely documentation of patient progress notes.
  • Promote quality and cost-effective interventions and outcomes.
  • Adhere to professional standards as outlined by protocols, rules, and regulations.
Qualifications:
  • Proven work experience in hospice care.
  • Effective communication skills.
  • Ability to work independently and as part of a team.
  • Excellent organizational and time management skills.
  • Strong clinical knowledge to manage complex patient needs.
  • Compassionate and empathetic.
  • Current LVN license.
  • CPR Certified.
  • Valid driver's license and reliable transportation.
