Community Support Specialist

Key Responsibilities:- Lead a small team of staff in supporting and advocating for clients, ensuring they receive quality care and achieve their goals.
- Develop and implement individualized support plans to help clients achieve maximum independence.
- Collaborate with clients, family members, and other stakeholders to ensure comprehensive support and advocacy.
- Provide training and guidance to staff on best practices in support and advocacy.
- Monitor and evaluate the effectiveness of support plans, making adjustments as needed.
- Ability to work weekends and evenings as part of a regional team to cover emergent needs.
- Pass TB and drug screen, as well as criminal history checks.
- Reliable transportation and ability to sit or stand for long periods.
- Ability to lift up to 50 pounds and grasp the equivalent of a 12 oz. can.
- High School Diploma or GED.
- Experience in social services, human services, or youth development.
