About the Role

Element Care is seeking a skilled Scoop Nurse Practitioner to join our team.

Job Summary

We are looking for a qualified professional with expertise in gerontology and experience working with older adults. The successful candidate will have strong assessment and consultation skills, as well as excellent communication abilities.

Key Responsibilities
  • Analyze complex health issues affecting older adults and develop effective care plans.
  • Conduct comprehensive assessments, including developmental status, mental status, social support, and nutrition.
  • Collaborate with primary care teams to address chronic illness and geriatric syndromes.
  • Assist patients and their families in formulating advance directives and end-of-life care decisions.
  • Frequent local travel required.
Requirements
  • Master of Science Degree in Nursing (MSN)
  • Current Board Certification as a Nurse Practitioner in the Commonwealth of Massachusetts
  • 2 years working with geriatric population or family practice
  • Experience working competently and comfortably with individuals from diverse socio-cultural backgrounds
  • Excellent computer and documentation skills
  • Covid vaccine required
