About the RoleThis is a full-time position that requires the successful candidate to work independently with minimal supervision. As a Home Health Clinical Services Coordinator, you will be responsible for providing home health clinical services to clients in their living environments.
Responsibilities- Assess Client Needs: Conduct initial evaluations of clients to identify their nursing needs and develop personalized plans of care.
- Deliver High-Quality Care: Provide wound care, administer medications, and perform other tasks as defined within the scope of nursing practice and state regulations.
- Collaborate with Healthcare Teams: Work closely with other medical disciplines, such as physical therapists and occupational therapists, to ensure coordinated services and comprehensive care.
- Maintain Accurate Records: Prepare clinical and progress notes, summaries of care, and maintain accurate records of client interactions.
- Communicate Effectively: Inform clients, families, and healthcare professionals of changes in client condition and needs, and provide education on disease management and prevention.
- Clinical Experience: Minimum two-three years of clinical experience, with at least half of that time working in home care or related healthcare organizations.
- Licensure and Certification: Current licensure and certification as a Registered Nurse in the State of Idaho, with wound care and IV certification preferred.
- Physical Demands: Ability to stand, walk, squat, bend, kneel, reach, twist, push, and pull, with varied assistance required depending on client ability, and ability to lift up to 50 pounds.
We offer a competitive salary range of $80,000 - $110,000 per year, depending on experience, plus opportunities for professional growth and development in a dynamic and supportive work environment.
