Job Summary

We are seeking a skilled Patient Care Coordinator to join our team at Kootenai Health. This is a challenging and rewarding role that requires strong communication skills, attention to detail, and the ability to work effectively with patients, families, and healthcare professionals.

Responsibilities
  • Assist patients in understanding their diagnosis, treatment options, and available resources.
  • Serve as an essential link between patients and all other medical staff.
  • Facilitate appointments for consults and support services within established service standards.
  • Coordinate multidisciplinary planning conferences and develop concise patient summaries for use by the care team.
  • Develop patient education programs and tools.
  • Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management resources.
  • Adhere to established productivity benchmarks and identify opportunities to reduce the cost of care.
  • Responsible for outreach efforts to establish and maintain positive working relationships with medical staff.
Salary Information

The estimated annual salary for this position is $65,000 - $85,000, depending on experience. Kootenai Health offers comprehensive medical plan options, including options for fully paid employer premiums for our full-time employees. For part-time employees, we offer the same plan options with affordable part-time premiums. In addition to medical insurance, we offer many voluntary benefits ranging from dental and vision to life and pet insurance. Benefits begin on the 1st of the month following 30 days of employment.
