Job Summary:

The LPN will be responsible for delivering and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician orders. This includes assessing patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate, and participating in the development and implementation of the nursing care plan.

Responsibilities:
  • Deliver and document skilled nursing care in accordance with physician orders and the developed care plan.
  • Assess patient condition and care needs, and report changes to the supervisor and/or physician as appropriate.
  • Participate in the development and implementation of the nursing care plan.
  • Provide education to patients and families regarding disease process, self-care techniques, and prevention strategies.
  • Maintain knowledge of competencies related to the nursing profession by participating in educational programs and continuing education units.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements:
  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
Environment:
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
