Hospice Nurse

Cantex Continuing Care Network is seeking a dedicated and compassionate Registered Nurse to join our team as a Hospice Nurse. In this role, you will provide high-quality nursing care to patients in their place of residence, working closely with the interdisciplinary team to coordinate care and ensure the best possible outcomes.
Responsibilities:
• Conduct initial and ongoing comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs
• Develop and implement individualized plans of care to ensure quality and continuity of care
• Educate patients and families on disease processes, self-care techniques, and end-of-life care
• Collaborate with the interdisciplinary team to prioritize patient needs and develop effective care plans
• Provide clinical direction to Hospice Aides and LPNs/LVNs to ensure quality and continuity of service
Requirements:
• Registered Nurse, licensed per state requirements
• 2 years of experience with a Hospice agency preferred
• Good communication skills, assessment skills, and strong clinical documentation skills
• Reliable transportation and valid auto liability insurance
