Overview:
At Armor Health, we are committed to providing exceptional patient care in a safe and supportive environment. As a Discharge Planner, you will play a critical role in ensuring that our patients receive the best possible care and support as they transition back into their communities.
Key Responsibilities:
- Monitor chronic care logs and caseload to identify patients who require additional support and referrals.
- Develop and implement discharge planning referral criteria for intake medical screeners.
- Collaborate with medical providers to identify patients who meet criteria for pre-release health planning.
- Consult with behavioral health providers to coordinate care for patients with complex needs.
- Participate in multidisciplinary planning to support specialized transitional programs for seriously or chronically ill patients.
Qualifications:
- Current/Active state license as an LPN.
- Ability to pass pre-employment Level 2 clearance, including a background check and drug screen.
- Minimum of two years' experience in a correctional health or criminal justice environment, preferably as a case manager, discharge planner, or nurse with similar responsibilities.
- Bachelor's Degree in social sciences, human services, or a healthcare-related field, or a Nursing Degree.
- Excellent written and verbal communication skills, as well as excellent computer skills.
- Familiarity with health records, medical terminology, and health information services.
