Key Responsibilities:

  • Monitor chronic care logs and caseload to identify patients who require additional support and referrals.
  • Develop and implement discharge planning referral criteria for intake medical screeners.
  • Collaborate with medical providers to identify patients who meet criteria for pre-release health planning.
  • Consult with behavioral health providers to coordinate care for patients with complex needs.
  • Participate in multidisciplinary planning to support specialized transitional programs for seriously or chronically ill patients.

Qualifications:

  • Current/Active state license as an LPN.
  • Ability to pass pre-employment Level 2 clearance, including a background check and drug screen.
  • Minimum of two years' experience in a correctional health or criminal justice environment, preferably as a case manager, discharge planner, or nurse with similar responsibilities.
  • Bachelor's Degree in social sciences, human services, or a healthcare-related field, or a Nursing Degree.
  • Excellent written and verbal communication skills, as well as excellent computer skills.
  • Familiarity with health records, medical terminology, and health information services.
