As a Case Manager II at Telecare, you will play a vital role in supporting individuals with severe mental illness (SMI) who are at risk of psychiatric hospitalization. Our intensive community support service, STRIDES of Alameda County, provides comprehensive care to 120 individuals between the ages of 18-62.

**Job Summary:**

We are seeking a highly motivated and compassionate individual to serve as a primary support and case manager for our clients. The ideal candidate will have experience working in mental health or community services and possess excellent communication and interpersonal skills.

**Key Responsibilities:

  • Serve as a primary support and case manager for individuals with SMI
  • Assist clients to live independently in their chosen community
  • Provide direct supervision and support to clients

**Requirements:**

  • High School Graduation or G.E.D. equivalent and two years of direct services in Mental Health or community services; or Associates degree and one year of experience; or Bachelor's degree in Social Services
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver
  • Willingness to use your personal vehicle to drive clients to appointments and groups

**Estimated Salary:** $21.00 - $23.71 per hour, depending on location and experience.
