Job Summary:

Optum Home & Community Care is seeking a skilled Healthcare Professional - Patient Connect Specialist to join our team. As a Patient Connect Specialist, you will play a critical role in ensuring a smooth transition from rehab to home for patients who have recently been hospitalized.

Key Responsibilities:

  • Perform comprehensive assessments and document findings in a concise and compliant manner.
  • Ensure accurate and timely documentation of diagnoses, medications, and other relevant information.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in quality initiatives and contribute to the development of new and improved methods and approaches to concerns/issues.

Requirements:

  • Certified Nurse Practitioner (NP) or Certified Physician Assistant (PA) through a national board.
  • Graduate of an accredited master's degree in nursing (MSN) program or accredited Physician Assistant degree program.
  • Active and unrestricted license in the state of practice.
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.

Preferred Qualifications:

  • 1+ years of hands-on post-grad experience within Long Term Care.
  • Understanding of Geriatrics and Chronic Illness.
  • Proficient computer skills, including the ability to document medical information with written and electronic medical records.
