Key Responsibilities:
  • Perform comprehensive assessments and document findings in a concise and compliant manner.
  • Ensure accurate and timely documentation of diagnoses, treatments, and patient outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in quality improvement initiatives and contribute to the development of new and improved methods and approaches.

Requirements:
  • Certified Nurse Practitioner or Physician Assistant through a national board.
  • Graduate of an accredited master's degree in nursing or physician assistant degree program.
  • Active and unrestricted license in the state of practice.
  • Current DEA licensure or ability to obtain post-hire, per state regulations.
  • Ability to travel to client and patient sites within a designated area.

Preferred Qualifications:
  • 1+ years of hands-on post-grad experience in long-term care.
  • Understanding of geriatrics and chronic illness.
  • Proficient computer skills, including documentation of medical information with written and electronic medical records.
