The UMC Health System is seeking a highly skilled and dedicated Staff Pharmacist to join our team. As a key member of our healthcare team, the successful candidate will be responsible for optimizing patient outcomes by providing the safest and highest quality of care to patients.
Key Responsibilities- Provide medication therapy optimization and management services to patients, including reviewing physician orders and patient profiles for standard and appropriate therapy.
- Ensure medication orders are processed in a timely manner and intervene as necessary to prevent medication errors.
- Collaborate with healthcare professionals to provide a seamless, multidisciplinary service and identify opportunities for process improvement.
- Develop and implement strategies to promote medication safety and adherence, including educating patients and healthcare professionals on medication use and management.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures related to medication management.
- Registration and licensure as a registered pharmacist by the Texas State Board of Pharmacy within the first 90 days of employment.
- Doctor of Pharmacy degree or Bachelor of Science Degree in Pharmacy.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, healthcare professionals, and other stakeholders.
- Strong analytical and problem-solving skills, with the ability to interpret complex data and make informed decisions.
The Staff Pharmacist will work in a fast-paced, dynamic environment with frequent interruptions and deadlines. The successful candidate must be able to work effectively in a team-based setting and adapt to changing priorities and circumstances.
